Incident reports are available to those affiliated with the incident for a fee of $27.00. Please complete this request form below.  If you have any other questions, please contact the Administrative Office at (916) 933-6623. 

*Note: If you are trying to request a Patient Care Report, you will need to contact El Dorado County EMSA as we do not retain these records.

PAYMENT

The fee for an incident report is $27.00 per our Fee Schedule. Payment can be made via PayPal or by check. Please select one of the payment methods below.

*Paying via PayPal: you will be redirected to PayPal's website after you submit this form. You will be charged a 3% processing fee.

*Paying by check: your report will not be processed until payment is received.  Make checks payable to El Dorado Hills Fire Department and send to EDHFD Re: Incident Report Request, 1050 Wilson Blvd., El Dorado Hills, CA 95762.

Please allow 10 business days for processing once payment is received.
